Empowering People To Lead Healthy, Dignified Lives. Our 245D Home and Community Based Services program helps people on a waiver live in their homes and maintain community independence. We serve people of all ages on these waiver programs, CADI, BI, DD, CAC and EW.

Our services are person-centered and are based on our clients' goals and needs. Each client has a care team that works together to support them achieve their goals, they include the lead agency case managers, our NorthStar Services managers, the client, their representatives, and our caregiver staff.

Northstar Support Services

We have an Integrated Community Supports (ICS) program with apartment units around the twin cities metro area. We serve and house clients with waivered services in this new integrated program.
